INSTRUCTIONS

STEP 1 - Answer the Questions
Answer each of the questions on the left as honestly as you can by putting a mark in the appropriate column. It will not help you to "fudge;" the purpose of this tool is to honestly evaluate your management skills. If some questions do not apply, try to reword them so that they are appropriate to your position and function. Do not skip questions.

STEP 2 - Score the Questions
When you have completed the questions, add up the total responses for each of the five columns. Put these total response numbers at the bottom of each column and multiply by the number in the respective box. For example, if you have a total of 10 "ALWAYS" responses, you will multiply 10 x 4 and put the result of 40 in the bottom box under the "ALWAYS" column. Do this for each of the five columns. Obviously, any responses in the "NEVER" column will yield a total number value of 0.

STEP 3 - Grand Total
Now add the bottom row of number values to yield a grand total score. Write that total here:
